Abstract
The Institute for Accountable Care has developed a Microsoft Excel-based tool to help ACOs and other providers estimate the potential return on investment (ROI) from implementing home-based primary care (HBPC) and community paramedicine programs. The tool allows ACOs to estimate the cost of operating their planned home-based care program and draws on the research literature to summarize the range of potential savings generated by these programs. Because the tool is designed for ACOs, the impact of HPBC programs on potential shared savings are a key aspect of estimating program return on investment.
